The Fund Terminate Request Form is used for all requests to terminate a foundation fund.

To close a fund, a Fund Termination and Transfer Form should be completed. The following information is necessary to process such requests:

  1. Fund number and name to be closed
  2. Fund number and name to which any remaining cash should be transferred or new funds deposited
  3. Reason for the fund being closed
  4. Fund´s Authorized User´s signature
